Case Manager, Geriatric

We are seeking innovative Case Managers with experience in the senior care system to serve as case managers in our program team. Through partnerships with insurers, Assured Allies will offer pre-claim support to aging adults and/or their family caregivers. The Ally Case Manager will provide virtual case management, support to families, and coordination of services (e.g med management, home care, home modifications, OT/PT). As an Ally Case Manager, you should be organized, able to take initiative, and have effective time management skills. You should also be open to a new approach to providing case management and comfortable working with a startup mentality. Our Ally Case Managers should be able to adapt to a changing approach, while challenging the team to develop creative solutions.

To the qualified candidate we offer a salary based on experience, between $45,000-$55,000, medical, dental, vision, PTO.


  • Work in a team environment to educate, plan, coordinate, implement, monitor and evaluate care and services to meet each member’s needs.
  • Case Management responsibilities
  • Complete phone assessments, create service plans, and coordinate services in service plans for a limited number of members.
  • Document all interventions, contacts, and work accomplished in our electronic records and billing system.
  • Provide ongoing support via phone, email, and/or video calls to members and their families.
  • Monitor and oversee home/community services to support successful aging in place.

Work with internal staff to research, identify, and provide resources and education to members and their families as needed.

Refine our services offering and policyholder engagement techniques

Provide ongoing support via phone, email, and/or video calls to members and their families.

Provide member- and Case Manager-generated feedback to make improvements to the Assured Allies program.


  • Must have a degree in either:
  • Social work or a related field.  Ideally LSW, MSW, LCSW or LICSW or comparable)
  • OT, LPN or RN with geriatric case management experience

At least 2-5 years’ experience working with older adults or adults with a need for some level of ongoing case management, chronic or progressive illnesses and the impact on families/caregivers.

Must have some knowledge of the elder  continuum of care..

Must be comfortable with working within and learning new software and technology tools that support aging in place.
