As the first step in our journey, we are working with long term care insurance (ביטוח סיעודי) companies, building new solutions for this huge but struggling market. We use advanced statistical and machine learning algorithms, unique data collection methods, and personalized, evidence-based clinical interventions. One of our products – the Age Assured program, which identifies near-future risks to policyholders and introduces customized interventions to mitigate them – is already live, while the construction and marketing of other products are well underway. A bit about the role:

Are you looking for a telephonic Case Management position where you have the opportunity to help and coordinate care for older adults? Are you looking to improve the health and quality of a senior’s life to enable them to successfully age in place?

We are looking for experienced Case Managers with experience in the senior care system to serve as case managers in our program team working from our Wellesley, MA office (This is currently a remote position. We follow the MA phases and guidelines – eventually work will be in our office located in Wellesley, MA).

You should be organized, and have effective time management skills. You should also be open to a new approach to providing case management and comfortable working in a startup culture; adapting to a changing approach while challenging the team to develop creative solutions.

To the qualified candidate, we offer a salary based on experience, between $50,000-$60,000, medical, dental, vision, PTO.

Responsibilities:

  • Work in a team environment to educate, plan, coordinate, implement, monitor, and evaluate care and services to meet each client’s needs.
  • Complete phone assessments, create service plans, and coordinate in-service plans for our national member base.
  • Utilize our electronic case management system to continuously document all client interactions.
  • Connect members to home/community services to support successful aging in place. Provide ongoing support for navigation and participation.
  • Work with internal staff to research, identify, and provide resources and education to clients and their families as needed.
  • Provide ongoing support via phone, email, and/or video calls to members and their families.

Qualifications:

  • Must have a degree in either: OT, LPN, or RN Social work (ideally LSW, MSW, LCSW or LICSW) or gerontology or comparable experience.
  • At least 2-5 years’ experience working with older adults or adults with a need for some level of ongoing case management, chronic or progressive illnesses, and the impact on families/caregivers.
  • Must have some knowledge of the elder continuum of care.
  • Must be comfortable working within and learning new software and technology tools.
